Hazard Identification Tracking System


HITS is an essential component of ASE’s Safety Management System. Think of HITS as an early warning system. Our Owners and Staff are on the front lines of the safety effort, and are in the best place to relay day to day safety concerns or ideas. Information obtained through HITS will be analyzed to see if there are areas that need extra attention, or things we could be doing better in the safety department.  However, the program will only work with your input.

HITS forms may be submitted over the internet:
  1. Log into MyFBO.
  2. Click the “Guidance” link in the upper left corner under the "Home" tab.
  3. Under “Hazard and Incidence Reporting”, select “Create Hazard/ Incident Report”.
  4. For type of report select “Hazard” for a situation you feel could lead to an accident or incident, “Incident” for a situation where the beginnings of an accident chain actually occurred.
  5. Your name should appear by default in the “Reported by:” field.  If you wish to report anonymously, simply delete your name, and no personal information will be transmitted with the report.
  6. Fill out the fields on the form and click “Enter”.


Click on the ASRS link above to visit the NASA Aviation Safety Reporting System website where ASRS forms can be submitted on-line.


Reporting an accident to the NTSB

Effective March 8, 2010, the NTSB now requires notification of a complete loss of information, excluding flickering, from more than 50% of an aircraft's cockpit display (PFD or other integrated display).  AOPA "Pilot Counsel" columnist John Yodice offers more insight in this brief article; click here to view.  More information can be viewed by clicking here.

 Fill in Form 6120.1  :  Pilot/Operator Aircraft Accident/Incident Report
  • Obtain the form from the requesting NTSB office or download it from this Web page.
  • If you download it from this page, you have two options:
    1. Print our Print-Only form and fill it in manually [click the link to download]
    2. Fill in our Fillable and Savable form and print it [click the link to download]
    • NOTE: You may save a copy of the the fillable form for subsequent editiing.
  • Sign the form and submit by FAX or mail.
  • To submit by mail, print your choice of regional Business Reply Mail (BRM) cover page 
    from the list below.

  • Each Business Reply Mail (BRM) includes a bar code. The bar code cannot be reduced in size or it will be rejected by the Postal Service. Therefore, you MUST print this page at 100%, without scaling.
