AVIATION EMERGENCY RESPONSE PLAN Click the icon to the right to download and view our proprietary emergency response plan. In it, AirShares Elite staff will find detailed, step-by-step procedures for handling a serious or fatal aircraft accident. The roles outlined here should be assigned and revisited whenever a change in personnel occurs. AirShares Elite staff will be drilled on these procedures on a semi-annual basis.

FIELDING THE CALL AirShares Elite staff: Print out a few copies of this PDF file on legal size paper (8.5" x 14"), fold, and stow in your car, office, home, etc. This template will facilitate gathering all the necessary information from the initial call so that AirShares Elite can react swiftly and efficiently. It will also serve as a checklist for gathering of pilot history data and aircraft maintenance records.

The NTSB references below are 2008 versions. Some of the regulations concerning notification and reporting requirements on aircraft accidents or incidents were recently amended, effective March 8, 2010. I will update this section as soon as the current regulations are posted.

- Obtain the form from the requesting NTSB office or download it from this Web page.
- If you download it from this page, you have two options:
- Print our Print-Only form and fill it in manually [click the link to download]
- Fill in our Fillable and Savable form and print it [click the link to download]
- NOTE: You may save a copy of the the fillable form for subsequent editiing.
- Sign the form and submit by FAX or mail.
- To submit by mail, print your choice of regional Business Reply Mail (BRM) cover page
from the list below.
